
INTRODUCTION
Advanced Life Support in Children (ÇİLYAD) Courses as a National Project was started in June, 2000. The purpose of this project is to reduce the rate of infant and child mortality and morbidity which is fairly high at present. Regarding this aim, the literature on this topic was examined and an appropriate program for our country was prepared.
The aims of the 2nd ÇİLYAD workshop which was held in Konya between 17-18 January 2008 were as follows:
To evaluate the targets of the year 2007
To evaluate the process in advanced life support in children and gain new experiences for the future applications, to obtain standardization and develop the program.
To discuss the problems faced during the courses and try to find solutions.
To comment and discuss the results of the courses
To plan the changes that will be applied in the new courses
To plan the courses to be given to the doctors and other health care workers in 2008
To generate ideas to improve the quality of the courses taking the advantages of the experiences and knowledge of the participants in different cities and part of the country.
LEGAL STATUS
Health Ministry started “Emergency Health Care Certificate Program” on 23rd January 2004. The scientific advisory board of Health Ministry, Emergency Health Care Office decided that Advanced Life Support in Children (ÇİLYAD) program to be one of the compulsory modules to be taken by all emergency health care professionals within the Certificate Proram.
PRESENT STATUS AND STUDIES
In order to start Advanced Life Support in Children (ÇİLYAD) courses throughout the country, Ministry of Health assigned Prof. Dr. Aytuğ Atıcı, from Mersin University Department of Pediatrics as the moderator of the course program. Dr. Turgut Arpacı was assigned as course coordinator in order to sustain coordination and to make necessary preparations for the courses.
ÇİLYAD provider and instructor books were prepared by various authors under the editorial of Prof.Dr. Aytuğ ATICI and published by Health Ministry.
Training program and plans for ÇİLYAD courses were prepared by the Basic Health Care Services Directorate and Emergency Health Care Department. The prepared program and related materials were distributed to the cities where the courses will be held. After the necessary substructure preparations on the courses were prepared, coordination and cooperation is provided by negotiating with those who are in charge of training in the cities that have emergency health care regional training, research and application center.
Within the scope of the training plan, first of all instructors were trained and those who were successful were certificated. After that with the help of the emergency health care regional training, research and application centers, the medical faculties and state hospitals ÇİLYAD courses have become widespread.
BETWEEN 2000 AND 2004
Before the Emergency Health Care Certificate Program introduced, a total of 7 courses were operated. Referencing the feedback from these courses, on 9-10 December 2004 the first ÇİLYAD was carried out at Mersin University Medical Faculty within the frame of Emergency Health Care Certificate Program. In this first course, the participants were from Adana and Mersin, the core instructor group who will carry out the training program in regional training offices were trained.

Based on the targets defined by the Ministry of Health instructor courses were held in Adana, Ankara, İzmir, Malatya and Mersin.
TARGETS AND SUGGESTIONS
It is vital that the training programs be prepared according to the latest and accessible data and be appropriate to our country’s facts in order to decrease the rates of child and infant mortality and the preventable injuries.
In order to improve the quality and efficiency of the care provided to the children, all the doctors and staff working in emergency health care centers should be trained on this topic and they should use it effectively and also the training courses should be standardized.
To do this: the work groups formed in the workshop and suggested that:
In order to evaluate the efficiency of the ÇİLYAD, an assessment should be done
Scenarios should be written and taught step by step.
3, 6 and 12 months after the training, tests should be conducted to obtain data for the upgrade in the future.
In order to learn about the efficiency of the course, mortality and morbidity rates in each region should be searched.
By establishing medical inspecting departments in emergency services of the hospitals and in ambulance services, applications should be inspected and the staff that was considered inadequate should be re-sent to training.
Case presentations should be in web site so that interaction can be provided.

Updating training should be done totally on case scenarios ( including instructors updating training as well)
The instructors should have practices on two course topics theoretically and practically besides their own specialty.
The lessons recorded on the video should be uploaded to the web page where the course participants can watch and upgrade their knowledge.
Standard control list should be prepared and uploaded to the web site
Case scenarios should be given to the course graduates periodically in order to evaluate the efficiency of the courses.
Monthly forms should be prepared for the province ambulance services and emergency services in hospitals to obtain statistical data (Was advanced life support used? Who applied it? Did the person take any training on this topic? When? Is the patient group mostly consisting of children?).
During the validity of the certificate a one day practice, case study and exams should be given ( at least once a year) to update the knowledge.
Studies should be conducted to compare the success of the staff who took the course to that of those who did not.
Reanimations should be recorded for the better planning and decreasing the mistakes.
As substructure emergency services and intensive care units should be supported.
To evaluate the course quality, the application of “supervisor” should be put into practice.
“Ventilation during the transport” should be added to the new edition of the book.
Following the fifteen days after the course, necessary information should be uploaded to the web site.
Based on the opinions of the instructors some changes on the web page should be done.
ASSESSMENT AND RESULT
At the end of the Workshop, the presentations made by the course directors, group works and discussions were evaluated. After the legal arrangement by the ministry of health at 2004 to the end of 2007, the target population of the ÇİLYAD courses was reached mostly. In order to spread the program to nationwide quickly, it was observed that the contribution of the instructors and universities were high. The courses throughout the country were held with the help of instructors from 29 medical faculties and it is stated that everybody was happy about the cooperation and the spreading speed of the sources observed increasing.
It has been observed that the required material for the courses was quickly supplied by the health managers of the provinces that have Regional Training Centers. In addition to this, it has been observed that the health directors in some cities that do not have Regional Training centers started to organize courses with the help of the medical faculties and obtaining adequate number of instructors.
Human resources, materials, physical environment, scientific and technologic support and coordination for the ÇİLYAD were found to be fairly good. On the other hand, in some places there has been a difficulty in using the financial resources as the authorities were unwilling to let them use the money and even sometimes that formed a barrier to organize courses. However, today we can see that most of these difficulties were cleared away and the number of the ÇİLYAD courses increased greatly.
The main topic of the “The 2nd Workshop and Course Moderator Training” is defined as “standardization”. Nearly 100 moderator and instructor from 24 cities took part in the workshop. Ideas are generated on the standardization of the courses nationwide and a decision was reached as the necessary study should start immediately within a “Quality Management System”
With the introduction of online registration system for ÇİLYAD courses a great step towards standardization was taken. For the control of the standardization some instructors were selected as “ ÇİLYAD director”. The directors who organized at least 10 ÇİLYAD courses were assigned as “ÇİLYAD Supervisor”
In order provide care not only to the children in our country but also in some neighbor countries, where ÇİLYAD is not introduced yet, translation of the web site into English was decided
The topic of the workshop to be held in 2009 is decided as “ASSESSMENT AND EVALUATION” and studies on this topic has started
